Complete a completed child enrollment file to include these documents:
- Enrollment Form
- Emergency Medical Authorization
- Parental Agreement
- Parent Acknowledgement Page
- Parent Notice of Liability Insurance
- Incident Report
- Infant Feeding Plan (if applicable)
- Authorization for Medication
- Vehicle Emergency Medical Form
- Transportation Agreement
- Field Trip Permission Form
*Parents must provide up-to-date immunization records by the date of expiration or your child will be unenrolled*
